The below is a list of the Society's practice forms.

Please note, the following requests are to be submitted via your dashboard:

  • CPD Extension Request 
  • CPD Exemption Request
  • Surrender of an ACT PC
  • Transfer of an ACT PC

To access your dashboard, you will need to login using the "Login/Register" button on the home page. We encourage you to submit your forms this way, where possible.
